Virtual Medical Evaluation
You are purchasing a professional virtual medical evaluation performed by a Michigan-licensed physician. If clinically appropriate, the physician will issue a prescription authorizing your use of a specific over-the-counter topical anesthetic during your procedure. StudioMeds, PLLC does not sell, dispense, or ship medication.
Price and Billing
Price: $35.00 per evaluation (one-time fee). No subscriptions. No recurring charges. Cash-pay only.
You will not be charged if the physician determines you are not medically appropriate to receive the prescription.
Terms and Conditions
You are purchasing a virtual medical evaluation, not a medication or product. If clinically appropriate, the physician may issue a prescription. Approval is not guaranteed. StudioMeds does not sell or dispense medication and is not a pharmacy. Procedures themselves are performed by separately licensed body art professionals; StudioMeds does not perform procedures.
You will not be charged if you are not medically appropriate. Once your evaluation is completed and the prescription has been delivered to your email address, the service is considered fully rendered and no refund will be issued, except for narrow circumstances described in the Refund Policy (such as duplicate charges or technical delivery failures).
By proceeding, you consent to electronic communications, including delivery of your prescription by email. Disputes are resolved by good-faith negotiation followed by binding arbitration under the American Arbitration Association's Consumer Arbitration Rules, except that you may bring an individual claim in small claims court if it is within that court's jurisdiction. Class actions are waived. Michigan law governs.
Privacy Policy
StudioMeds is operated by a Michigan-licensed physician and is subject to Michigan physician confidentiality law and the Michigan Medical Records Access Act. We collect identification, health, and limited payment confirmation information necessary to conduct your evaluation, issue and deliver your prescription, and maintain the medical records that Michigan law requires us to keep.
We do not sell your information. We share your information only with the prescribing physician, with a limited set of service providers required to operate the platform (each contractually bound to protect your information), with the body art facility as required by Michigan body art regulations (this happens at your direction when you provide the prescription to your artist), and with regulatory or legal authorities when required by law.
You have the right to access your records, request correction of inaccurate information, request a copy in portable format, and request reasonable restrictions on disclosure. To exercise these rights, contact admin@studiomeds.com.
In the event your information is acquired by an unauthorized party, we will notify you consistent with Michigan's Identity Theft Protection Act.

Patient Consent for Clinical Intake and Treatment
You are completing a clinical intake questionnaire so a Michigan-licensed physician can determine whether you are medically appropriate to use a specific over-the-counter topical anesthetic during your procedure. Approval is not guaranteed. The physician evaluates your eligibility for the medication only and does not perform your procedure.
You confirm that all information you provide is true, accurate, and complete. The medication, if prescribed, must be used only as directed. Excessive use, combining anesthetic products, prolonged application, occlusion not authorized by the prescription, or use on broken or compromised skin not authorized by the prescription can cause serious harm, including symptoms such as numbness around the mouth, metallic taste, lightheadedness, irregular heartbeat, seizure, or loss of consciousness. Seek immediate medical attention if you experience any of these symptoms.
For lip blush and permanent eyeliner procedures, the prescription is for off-label use of an FDA-regulated over-the-counter topical anesthetic, off-label specifically as to the anatomical site of application. Off-label use of FDA-regulated medications by a licensed physician is a recognized and lawful element of medical practice. Procedure-specific risks are described in detail in the full Patient Consent document, including elevated absorption through lip tissue, cold sore reactivation for lip procedures, and risk of product migration into the eye for eyeliner procedures.

Affiliate Relationships Disclosure
The physician's prescription email may include affiliate links to retailers (including the StudioMeds Amazon storefront) where you can purchase recommended products. If you purchase through an affiliate link, StudioMeds may receive a commission from the retailer. The commission does not affect the price you pay. The clinical recommendation is independent of any commission structure. Your prescription is complete and valid regardless of where you purchase the recommended product.

I acknowledge that I am purchasing a virtual medical evaluation, not a medication or product, and that a prescription will be issued only if clinically appropriate. I have read and agree to the Terms and Conditions, the Privacy Policy, the Patient Consent for Clinical Intake and Treatment, and the Refund Policy. I understand that I will not be charged if I am not medically appropriate, and that once my prescription is issued and delivered, no refunds are provided except in the narrow circumstances described in the Refund Policy. I consent to the collection, use, and secure handling of my information and to receiving electronic communications including delivery of my prescription.
